Direct Care Professional
Direct Care Professionals provide individual support and training to residents with intellectual disabilities and sensory impairments. Direct Care Professionals work in the community homes and Habilitation Specialists work in the Day Program workshops. Various day, evening, overnight and weekend shifts are available in Philadelphia and Delaware County. Diploma/GED and valid driver's license is required. Experience preferred.
Medical Coordinator
The Medical Coordinator acts as the liaison with physicians and is responsible for administering medications, monitoring residents, and providing assessments and follow-up care. Applicants must be an RN or LPN. This is a Tuesday — Saturday position. Full benefits, no holidays, shared on-call responsibilities with Nurse Manager, and free on-site parking available.
